John Doe, General Manager
ABC Bingo Hall
123 Brown Road
Anywhere, Florida 12345
June 1, 2017
Mr. Doe:
The ABC Bingo Hall denied me and my service dog entry on May 25, 2017 to its bingo games held at 123 Brown Road. Immediate action is requested to address this specific situation and to prevent future occurrences.
When I attempted to enter ABC Bingo Hall at 10:00 am, your employee Jane Smith, stated I could not come in with my service dog. I tried to show her the Department of Justice ADA Service Dog FAQ guidance document. During this 5-minute conversation, my service dog remained laying down, non-vocal and under my control on a leash. Despite my efforts, she refused to look at the Department of Justice or Florida service dog laws. I asked to speak to a manager. She said “I’m the manager on duty” and again refused to let me in.
On May 25 and 26 I left voicemail messages at 123-456-7890 asking for a call to discuss this matter. I have not heard back from anyone. On May 27, I went to your Brown Road location and asked to speak to someone in charge to discuss this matter. Donnie Blue told me no one was available. After providing my contact information and a copy of the Department of Justice’s Frequently Asked questions about Service Animals and the ADA and a copy of Florida Statute 413.08, I asked Mr. Blue to have someone contact me. I have heard from no one.
I want to come regularly with my service dog and play bingo. I look forward to discussing this matter in person with the ABC Bingo Hall to determine how we can resolve this. Please contact me no later than June 11, 2017.
Sincerely,
Tom Sample
